Job Summary
The Rehabilitation Coordinator oversees and facilitates rehabilitation programs designed to support individuals with disabilities, injuries, or chronic conditions in achieving their goals of improved functionality and independence. They serve as the primary liaison between clients, healthcare providers, and support personnel, ensuring that rehabilitation services are delivered efficiently and effectively. This role requires exceptional organizational, communication, and problem-solving skills intended to manage individualized care plans and ensure seamless program execution.
Primary Responsibilities
Assess the needs of clients and develop tailored rehabilitation plans in collaboration with healthcare professionals.
Coordinate a multidisciplinary team, including therapists, physicians, and case managers in an attempt to ensure comprehensive care.
Schedule and manage rehabilitation sessions, tracking client progress and addressing any barriers to success.
Act as the main point of contact for clients and their families, providing guidance, resources, and emotional support.
Maintain accurate records of rehabilitation plans, progress reports, and program evaluations.
Ensure compliance with legal, ethical, and regulatory requirements in all rehabilitation services.
Stay current with best practices, tools, and technologies in the field of rehabilitation to continually improve service quality.
Advocate for clients' rights, accessibility, and inclusion in various environments, including workplaces and communities.
Knowledge and Skills
Strong organizational, project management, and coordination abilities.
Excellent communication and interpersonal skills to work with a variety of assorted personnel.
Proficiency in using case management software, adaptive technologies, and other relevant tools.
Empathy, cultural sensitivity, and a client-centered approach to problem-solving.
Minimum Qualifications
Bachelor’s Degree in Rehabilitation Science, Social Work, Healthcare Administration, Occupational Therapy, or a related field.
Minimum of 2 years of experience in rehabilitation services, healthcare, or case management.
