The DHS/DDS HCBS Program Coordinator assigned to this units principal duties include: review assigned incident from the DDS Incident Management system,collect incident information by exchanging information with DDS units,other DHS division, PASSE , and Providers. Analyze incident reports to determine if additional information is needed and the dissemination for investigation. Enter incident data into Salesforce. Monitors eligibility through ARIES MMIS as needed ; to verify members enrollment status in HCBS programs or service interruption .Monitors incidents caseload to include ;tracking and follow-up ,for timely incident submission according policy and the nature of the incident ,evaluates incident reports while ensuring compliance by providing technical assistance to other divisions/agencies until closure of the incident. Reviews records and information for Community Mortality review all related to unexpected or unexplained death of individuals served by community programs to include the CES Waiver members, which includes requesting official Death Certificate,and additional information from HCBS providers.
