Job Summary: This position will be responsible to promote Consumer Choice and facilitate Nursing Home Transition to a least restrictive environment.
*Responds to Nursing Home Options Counseling resident referrals, and other approved resident referral sources, within an established time frame to provide face-to-face contact.
*Gather information about the residents’ situation to assess and offer community resource options.
*Provides Community Resource information to ensure an informed Consumer Choice of Long-Term Support Services that promotes independent living functions and the feasibility of a move to a least restrictive environment.
*Interacts directly with nursing facility representatives in explaining the service of Community Resource Options Counseling information provided to nursing home residents.
*Promotes and provides Long Term Support Services and expands the Continuum of Care for Long Term Care.
*Serves as client advocate and assist with filling out related paperwork and submitting appropriate applications for services when requested by client and/or client representative.
*Acts as a division liaison with nursing homes to inform nursing facility or the client of the forms and documentation needed for determination of eligibility.
MUST HAVE:
Valid Arkansas driver's license
Case worker experience
Reliable transportation-frequent in state travel
