DYS   Division of Youth Services, Case Coordinator

 

JOB DUTIES & RESPONSIBILITIES

Face to face visits with the youth while in assessments, while conferencing in the guardian over the phone. Monthly face to face visits at program. Monthly phone calls to guardians, Juvenile Probation Officer (JPO) and After Care Worker (ACW). Building strong relationships with the youth and families.  Attend Treatment Team Meetings. Provide information to all parties involved in the Treatment Team meeting with information through the monthly Case Coordinator form. Maintain regular contact with Treatment Team Members on Case Coordinator roles and contact information.  Monitor compliance of treatment plan goals. Complete monthly Case Coordinator Treatment Team Progress report. Review progress reports from contracted programs to determine youth’s progress and treatment needs. Perform monthly face to face visits with the youth and call guardians. Complete Quality Assurance (QA) form while at the programs and submit findings to QA Manager and Case Coordinator Manager.  Attend/Schedule staffing’s with providers as coordinated and scheduled and/or directed when necessary.  Maintain compliance with DYS/DCFS cooperation agreement and interdivisional on any DCFS youth as required. Attend/Scheduled interdivisional as required.  Obtain and review information necessary for completing and submitting all regular and specialized treatment and required procedure packets for the following: Adolescent Sexual Assessment Project (ASAP), Extended Juvenile Jurisdiction (EJJ) packet, Interstate Compact packet (ICJ), prepare dual custody staffing reports (DCFS) and Registration Risk Assessment (RRA).  Review/submit discharge summaries and aftercare plan as a completed packet to be reviewed by the clinical director.  Tracking and initiating commitment order extensions, when necessary, on youths.  Contacting the courts for courts hearings or notifications for a youth’s release. Serve as a liaison with court officials, contracted programs, guardians, and any stakeholders for the youth’s case. Provide preceptorship to new employees as assigned. Complete discharge packet and checklist. Coordinator with Optum Independent Assessment and PASSE agency. Work with Community Base Providers to provide services for youth in the community once they finish treatment.

Meeting with CC Supervisor and corresponding with DHS Physician to work on youths over current Length of Stay (LOS), providing paperwork along with follow-up questions that are needed to be provided to the responsible parties.  Attend monthly Case Coordinator meetings as scheduled by CC Supervisor.  Keep Outlook calendar updated as far in advance as possible. Inform CC Supervisor of all appointments off of campus, sick and annual leave.  Perform other duties as assigned.

 

SPECIAL REQUIREMENTS

*  Ability to conduct a motivational-style interview with youth that have experienced trauma.

*  The ability to communicate professionally with the Court Systems.

*  Knowledge of the Juvenile Justice System and PREA Standards.

*  Ability to work independently, meet deadlines, as well as work in a positive team environment and atmosphere. Communication is very important in this position.

*  Must have a valid driver's license, current auto insurance and meet the eligibility requirements to be covered by the state insurance policy.  Must be able to operate a motor vehicle safely and according to applicable laws.

Job Summary

The Social Services Advisor provides guidance, counseling, and education to individuals and families receiving government assistance. This role involves assessing needs, recommending services, and advocating for clients in a variety of settings.

Primary Responsibilities

Advise clients on available state resources and support programs. Conduct needs assessments and recommend interventions. Provide guidance on job training, financial aid, and social service programs. Assist in crisis intervention and family support services. Collaborate with caseworkers and service providers to enhance client outcomes.

Knowledge and Skills
